Team Dinner/Meeting - March 21st 6:30pm

Please email me the number of guests attending this dinner/meeting-- it is important to have at least (1) parent at this event.   Players/siblings are invited-- but we are limited to 80 in this room.

1)  Birth certificate: email or give me a copy at our March 21st meeting
2)  School Report Cards: email me a copy after school ends
3)  Parent Medical Consent Form: each parent and doctor signature required
4)  Athlete Code of Conduct:  player signature required
5)  Parent Code of Conduct-- each parent signature required 
6)  League ID Cards and player pictures-  Lisa Vitale will be taking these pictures at our first practice.
